Job Description:
Provides advocacy services for clients and their families to include obtaining information identifying needs/barriers and problem solving.
Provide in-home support services and related activities while maintaining confidentiality as required by licensing, HIPPA and other regulatory agencies.
Collaborates as a member of a multi-disciplinary team to address the needs of the consumer and community services link-up.
Documents all activities in accordance with regulations and within requirements of D'Veal Family & Youth Services and as required by funding sources.
Good verbal and written communication skills.
Computer skills.
Attend training meetings and case conferences.
Drives personal vehicle as necessary.
Requirements Parent of a child who has had involvement with the Department of Children and Family Services and/or Probation or mental health systems.
Has knowledge of the Juvenile Court System.
