00 PM M-F ? Non-Exempt Position Summary Responsible for conducting home delivered meals program evaluations as part of the Elderly Nutrition Program (ENP) for elderly clients based on an assessment of their needs and consistent with all applicable policies and procedures.
Essential Duties and Responsibilities The essential functions include, but are not limited to the following:
Perform Home Delivered Evaluations and other related program duties as assigned for seniors and other eligible clients.
Process intakes; make referrals for other services; provide information; complete assessments to determine client needs; document case records, handle and resolve emergency situations, check progress towards identified client needs and goals; and ensure effective quality and service delivery.
Identify needs for elderly clients with respect to practical problems such as social security, housing, budgeting, home delivered meals, planning, and medical needs; provide information (e.
g.
, bus schedules) or direct assistance (e.
g.
transportation) to help elderly meet identified needs; 03 trouble shoot on behalf of client.
Monitor and assess clients' progress through frequent contact with client and/or community service providers.
Develop and maintain information about community services/agencies relevant to the clients' needs; refer, contact, or act on behalf of client with appropriate referrals and communication to agency staff.
Perform all associated record-keeping which includes but not limited to:
maintain a system for performing individual assessments for the home delivered meals program, perform all related services, and conduct periodic re-assessment ensuring accuracy and completeness Maintain work hours within the budget and completes forms documenting same; complete assignments, care management performance goals and documentation according to contractual funding requirements.

Participate in staff meetings and in-service training to develop/improve care management skills and to coordinate activities and services.
Maintain confidentiality in all matters of agency and program personnel, operations and clients; and maintain professional relationship with clients at all times.
Communicate and work with home delivered meals program staff to ensure open lines of communication and seeks advice and guidance on all home delivered meals and senior service-related issues and procedures.
Facilitate a team approach in working with other staff in order to ensure consistent and well-organized management of the YWCA Senior Services Program and related service systems.
Treat clients, family members, service providers, and co-workers with respect and dignity Support the mission and philosophy of the YWCA which includes acceptance of multicultural diversity.

Qualifications (Knowledge, Skills, and Abilities) BA degree in Psychology, Sociology, Human Services, or related field.
Current California driver's license and a driving record acceptable to the Agency's insurance carrier.
Bilingual skills in English and Mandarin and/or Cantonese.
Significant paid or volunteer experience in care/case management or a related field; or previous elderly nutrition program work experience.
Knowledge of human behavior and the aging process, sensitivity to issues affecting older persons, their families and the community.
Knowledge of community resources and available funding sources.
Knowledge of social and health intervention techniques.
Knowledge of information and referral programs and the ability to develop and expand them.
Ability to work effectively with volunteers, small groups, advisory boards, and the community.
Excellent verbal and written communication skills The ability to type 50 WPM accurately; proficient in use of 10-key and other office equipment; intermediate computer skills including experience with Microsoft Office products, including but not limited to Word, Excel, and Outlook; ability to learn and use the PC hardware and software.
Proficient in English grammar and spelling with excellent oral and written communications Willingness to attend training seminars and meetings outside normal working hours.
